The manga begins with a teenage Boruto Uzumaki facing an enemy named Kawaki during the destruction of his village, followed by a retelling of the events in Boruto: Naruto the Movie with added content. The son of the Seventh Hokage Naruto Uzumaki, Boruto is angry that his father puts the village before his family.

During this time, Boruto becomes a member of a ninja team led by Naruto’s protégé Konohamaru Sarutobi, along with Sarada Uchiha, daughter of Sasuke and Sakura Uchiha, and Mitsuki, Orochimaru’s artificial son. Sasuke returns to the village to warn Naruto of the looming threat to discover Kaguya Ōtsutsuki’s motives. Boruto asks Sasuke to prepare him for the upcoming Chunin Exams in order to impress his father.

During Momoshiki and Kinshiki Ōtsutsuki’s exam, the duo Sasuke met kidnap Naruto so they can use Kurama, the tailed beast sealed inside his body, to revive the dying Divine Tree from the dimension they came from. Boruto, Sasuke, and the four Kage, leaders of the other ninja villages, set out to rescue Naruto.

The battle ends when Momoshiki, sacrificing Kinshiki to increase his power, is defeated by Boruto and Naruto with the help of Sasuke; Momoshiki survives long enough to realize Boruto’s full potential, warning him of future hardships. After recovering from the fight, Boruto decides to become like Sasuke in the future, instructing Sarada to follow her dream of becoming the next Hokage.

On the next mission, Boruto serves as a bodyguard to the son of the Fire Daimyō Tento, and the two find a kinship in both wanting their fathers to recognize them. When a group of thugs known as the Mujina kidnap Tentō, Boruto saves him and the leader of the group is imprisoned due to his knowledge of the mark Momoshiki placed on Boruto.

Naruto and the others learn that there is a group called “Kara”. Husk is looking for people with a mark called Karma. Boruto’s team meets Kara’s fugitive Kawaki, a boy who also has Karma. Kawaki becomes an adopted member of the Uzumaki family in order to protect him. However, while trying to protect Kawaki, Naruto and Sasuke are defeated by Kara’s leader, Jigen, who seals Naruto while Sasuke escapes. Team 7 saves Naruto when Boruto’s Karma makes him possessed by Momoshiki.

Upon learning this, Sasuke discovers that all Karma users will be captured by the Ōtsutsuki clan, including Jigen. Meanwhile, a mutiny begins to form in Kara with Koji Kashin challenging Jigen while Amado travels to Konoha seeking asylum in exchange for information, revealing Kara’s real leader as Isshiki, who has been in possession of Jigen ever since he was betrayed by Kaguya. when they came to Earth millennia ago, and that Karma allows the Ōtsutsuki clan to be resurrected through the host’s body.

Although Koji kills Jigen, forcing Isshiki to imperfectly reincarnate while Kawaki’s Karma is removed in the process, Isshiki forces Koji to retreat and leaves to attack Konoha.

There, Isshiki searches for Kawaki. Naruto comes face to face with him, preparing to fight. Boruto transports himself and Isshiki to another location away from the village, with Sasuke and Naruto following him. Since Boruto is Momoshiki’s vessel, Isshiki plans to feed him to his Ten-Tails to plant the Divine Tree. With Naruto’s new power, the Baryon Mode, he defeats Isshiki before being tricked by Kawaki, causing Isshiki to die, but at the cost of Kurama’s life.

Boruto, possessed by Momoshiki, pierces Sasuke’s Rinnegan. However, Sasuke and Kawaki are confronted by Momoshiki until Boruto restores his body. After being defeated, Isshiki asks Koda, who was guarding the Ten-Tails, to fulfill the will of the Ōtsutsuki by sacrificing Boruto or Kawaki to the Ten-Tails, eat the chakra fruit, and become the Ōtsutsuki himself.

Koda vows to avenge Isshiki by going after Naruto, Sasuke, Kawaki, and Boruto. He visits Boro’s hideout and effortlessly kills the guards. He then releases one of the strongest cyborgs created by Amado to be disposed of.

The cyborg woman Eida agrees to help Koda kill Naruto if he in turn spares Kawaki so she can have a normal romance because her power of seduction prevents her from experiencing true love. Since her powers don’t affect the Ōtsutsuki, and Kawaki and Boruto are already part of them, she can experience it with them.
